💊☀️VITAMIN D3 SUPPLEMENTATION 👉🏽

I find my patients from all races and walks of life are almost always significantly deficient when I check their Vitamin D levels, whatever their diet unless they are supplementing.

Food is not a good or reliable source of vitamin D. Check for sources such as soya milk fortified with Vitamin D or sunbathe your mushrooms when you can.

Read my post from a couple of days ago about sunshine, which is sadly lacking for most of the year. 

Vitamin D is made in our skin when exposed to UVB rays of sunshine. 

REMEMBER TO COVER UP

Do not expose your skin in sunlight for prolonged periods of time and use sun protection and cover up after the initial timed exposure (15-45 min depending on the colour of your skin) to absorb the UVB rays to make Vitamin D. This is to reduce the risk of skin damage and skin cancer. (NHS guidance)

Vit D3 (800 -2000 IU daily with the largest meal of the day) is recommended during winter months, especially in northern latitudes. If in doubt or you don’t want to take supplements, check your levels regularly to ensure adequate levels. You may need higher doses in some situations under medical supervision. 

We discuss the role of Vitamin D and PCOS in our book Living PCOS FREE as well as the role of other supplements. 

Vitamin D deficiency is common in women and AFAB with PCOS, and there is some evidence that vitamin D supplementation after an initial blood test to measure levels may improve reproductive function and insulin sensitivity. 

Low Vitamin D levels can increase the risk of Covid-19 infection including in those with PCOS.

Vitamin D is best known for its critical role in bone health. Vitamin D (of which Vitamin D3 is most important) is more a hormone rather than a vitamin and has a number of functions involving calcium, phosphate and magnesium metabolism but also plays a role in gene regulation when it comes to many autoimmune diseases, cancers and infection.

I love starting my day with a plant-based breakfast such as porridge with soya milk and berries, tofu scramble with veggies or a smoothie. These are packed with dietary fibre and protein, omega-3 fats (I always include 1-2 tbsp ground flaxseed) and other micronutrients. I try to enjoy any decaf or caffeinated drinks 1-2 hours after breakfast as these can interfere with iron absorption.

A small study in 2015 found that women who regularly skipped breakfast had a disrupted cortisol (stress hormone) rhythm and elevated blood pressure compared to women who were breakfast eaters. The authors concluded that if this became a habit, it could increase the risk for cardiometabolic diseases such as type 2 diabetes in some people.

If you have Polycystic Ovary Syndrome (PCOS), it is especially important to choose a breakfast that keeps your energy levels consistent. FLESH EATING DISEASE: 👉🏽
☑️Necrotising fasciitis is a serious bacterial infection which can be life threatening but is fortunately very rare. Some groups are more susceptible. It’s not really a flesh eating disease but toxins released into the surrounding tissues by the pathogenic bacterial bugs breaks down the surrounding tissue (skin, muscle, fascia)

☑️I was asked to comment on this particular case by Cosmo. 
A rare occurrence, one which I have never seen in this scenario, having cared for thousands of patients. A scary experience nonetheless for the person concerned but people don’t need to worry about avoiding disposable pads for this reason. 

🧴My advice is to always remember to follow hand washing, frequent change and being aware of any background conditions that can cause infections to become more serious quickly such as if living with type 2 diabetes, cancer or if one is on certain medications or immunity is suppressed.

☑️Also, for anyone, don’t ignore a rash or redness or swelling if spreading, or accompanied with fever or feeling unwell or if one is concerned.

✅ However, if you are thinking of switching, consider one of the more environmentally friendly menstrual products on the market such as period pants which are brilliant and my patients love them (@wukawear do a great product: I don’t get paid to say this) or a moon cup for example.

🌱If you are on a plant based diet, ensure adequate Vit B12 intake, ideally with cyanocobalamin, which is more shelf stable than the natural form methylcobalamin, but either is fine. Also make sure you are not folate deficient and if needed, take a supplement that has both (green leafy veg, peas, chickpeas, brown rice, broccoli are some foods rich in folates). 

🌱If you are choosing not to supplement, then check your blood levels regularly (at least once a year) as the effects of Vitamin B12 deficiency can be slow to appear in adults but disability can be disastrous and permanent. Also, just taking a folic acid supplement without adequate B12 can mask symptoms of B12 deficiency. 

✅Vitamin B12 (a water soluble vitamin) supplements are cheap and safe and doesn’t need the close attention if one is trying to supplement only through foods (2-3 times a day of fortified foods).

✅Even omnivores (40%) can be b12 deficient, especially with increasing age. Supplementing after the age of 50 in this group is recommended by some experts.

Our Book Launch on Thursday for Living PCOS Free was more than just an event to promote our book. 

💛It was an occasion, a chance to bring together friends and like minded people we had not seen for sometime because of the pandemic.

The Hearth @thehearth.me a co-working space in London for women to live, work and feel better was the perfect and the most gorgeous venue with warm welcoming staff with great attention to detail. The best mocktails too.

💛The canapés by Chef Rishim and his dynamic team @chefrishimsachdeva 
were high end plant-based bites of heaven at affordable prices. Highly recommend visiting his restaurant @tendril_kitchen in London.

💛What made the evening so memorable for me was that Rohini had invited such a beautiful community of well wishers and positivity that evening. A close friend of ours said it was a collision of so many worlds: family friends, school friends, university friends, influencers and the plant based community, everyone there to support and celebrate our achievement. I arrived at my own book launch as a guest and was not disappointed. Rohini had taken care of every single detail. 

It was an evening where we both felt lifted by the love and warmth in the room. The energy was infectious and everyone had a wonderful time meeting old friends and making new ones. We had both realised our dreams of being published authors while getting to spread such an important message about PCOS. 

💛It was a wonderful and emotional day and I hope you enjoy some of the photos I have shared here. Some very dear to us could not attend but were with us in spirit.

Like most health professionals, I have really tried during the course of my career to deliver the best medical care I could to help patients get better. 

However, it is also important to be taught the science of application of lifestyle and nutrition alongside medications and surgery and getting to the root cause of any disease process. There is enough scientific evidence that lifestyle and nutrition play a very important role in preventing and managing many health conditions, including reproductive and hormonal health.

Applying all aspects of lifestyle alongside western medicine as needed can really make a great difference to the overall outcome to the health of the patients we treat.

It really doesn’t have to be one or the other. I wish more medical schools and more doctors would embrace this more whole heartedly. 

This is the approach applied throughout when @rohinibajekal and I wrote 
Living PCOS Free.

REPRODUCTIVE FREEDOM  is a human right, not a choice. PLEASE PLEASE AMPLIFY

The news about abortion laws being possibly overturned by the US Supreme Court has caused me turmoil.

It is my duty to speak up as an ObGyn who has seen pregnancy-related horrors during my long career and the impact of what happens when choice is taken away.

Pregnancy is a dangerous business, both a mental and physical challenge for many, with disastrous consequences when things go wrong.

Surely, anyone with a uterus should be allowed to decide for themselves whether they wish to carry a pregnancy or not.

If we take away safe choices, they are backed into a corner and left with no choice except an unsafe one.

Let me share with you just one of the many events I have witnessed. 

It has left an indelible scar in my heart and has influenced my practice deeply over the last 35 years. 

In my first year as a young ObGyn resident in India, I was called in the middle of the night to an emergency.

A 18 year old law student carrying twins around 4 months pregnant was in a critical condition. She was cold and clammy with a weak thready pulse and her blood pressure virtually unrecordable. 

I immediately called for senior help who arrived right away to resuscitate her. 

She was bleeding profusely from the vagina. My heart sank as my fingers felt slippery tube like structures coming away in my hand. 

Even though I had never come across anything like this, I knew I was feeling her intestines in my fingers.

Her uterus had been perforated in a back street abortion and the bowel pulled out of the womb in a botched job. 

To avoid shame to the family, her father had dragged her for a backstreet abortion to get rid of the pregnancy. These usually go horribly wrong with terrible outcomes. 

Sadly, the young woman did not survive despite a ten hour operation and died from septicaemic shock.

A life incomplete, her potential never realised, all because someone other than her thought they had control over her body.

What if she had been me or my sister, my daughter, my friend?

Even when the foetus or woman isn't in danger, it should ALWAYS be the WOMAN’s choice.

💊☀️VITAMIN D3 SUPPLEMENTATION 👉🏽

I find my patients from all races and walks of life are almost always significantly deficient when I check their Vitamin D levels, whatever their diet unless they are supplementing.

Food is not a good or reliable source of vitamin D. Check for sources such as soya milk fortified with Vitamin D or sunbathe your mushrooms when you can.

Read my post from a couple of days ago about sunshine, which is sadly lacking for most of the year. 

Vitamin D is made in our skin when exposed to UVB rays of sunshine. 

REMEMBER TO COVER UP

Do not expose your skin in sunlight for prolonged periods of time and use sun protection and cover up after the initial timed exposure (15-45 min depending on the colour of your skin) to absorb the UVB rays to make Vitamin D. This is to reduce the risk of skin damage and skin cancer. (NHS guidance)

Vit D3 (800 -2000 IU daily with the largest meal of the day) is recommended during winter months, especially in northern latitudes. If in doubt or you don’t want to take supplements, check your levels regularly to ensure adequate levels. You may need higher doses in some situations under medical supervision. 

We discuss the role of Vitamin D and PCOS in our book Living PCOS FREE as well as the role of other supplements. 

Vitamin D deficiency is common in women and AFAB with PCOS, and there is some evidence that vitamin D supplementation after an initial blood test to measure levels may improve reproductive function and insulin sensitivity. 

Low Vitamin D levels can increase the risk of Covid-19 infection including in those with PCOS.

Vitamin D is best known for its critical role in bone health. Vitamin D (of which Vitamin D3 is most important) is more a hormone rather than a vitamin and has a number of functions involving calcium, phosphate and magnesium metabolism but also plays a role in gene regulation when it comes to many autoimmune diseases, cancers and infection.

Thank you @sheerluxe for sharing my favourite breakfast ideas (link in bio to read the full article). I love starting my day with a plant-based breakfast such as porridge with soya milk and berries, tofu scramble with veggies or a smoothie. These are packed with dietary fibre and protein, omega-3 fats (I always include 1-2 tbsp ground flaxseed) and other micronutrients. I try to enjoy any decaf or caffeinated drinks 1-2 hours after breakfast as these can interfere with iron absorption.

A small study in 2015 found that women who regularly skipped breakfast had a disrupted cortisol (stress hormone) rhythm and elevated blood pressure compared to women who were breakfast eaters. The authors concluded that if this became a habit, it could increase the risk for cardiometabolic diseases such as type 2 diabetes in some people.

If you have Polycystic Ovary Syndrome (PCOS), it is especially important to choose a breakfast that keeps your energy levels consistent. I have lots of breakfast recipes in our new book Living PCOS Free including an anti-inflammatory turmeric tofu scramble, overnight oats with stewed berries and chickpea flour pancakes 🌱

DON’T MESS WITH VITAMIN B12👉🏽

🌱If you are on a plant based diet, ensure adequate Vit B12 intake, ideally with cyanocobalamin, which is more shelf stable than the natural form methylcobalamin, but either is fine. Also make sure you are not folate deficient and if needed, take a supplement that has both (green leafy veg, peas, chickpeas, brown rice, broccoli are some foods rich in folates). 

🌱If you are choosing not to supplement, then check your blood levels regularly (at least once a year) as the effects of Vitamin B12 deficiency can be slow to appear in adults but disability can be disastrous and permanent. Also, just taking a folic acid supplement without adequate B12 can mask symptoms of B12 deficiency. 

✅Vitamin B12 (a water soluble vitamin) supplements are cheap and safe and doesn’t need the close attention if one is trying to supplement only through foods (2-3 times a day of fortified foods).

✅Even omnivores (40%) can be b12 deficient, especially with increasing age. Supplementing after the age of 50 in this group is recommended by some experts. 

✅ Medications like metformin used in PCOS or type 2 diabetes can deplete B12 stores over time, so supplementation is important.

✅ Very occasionally, a few people need B12 injections (hydroxocobalamin) in the presence of medical conditions that reduce absorption of b12. Your doctor should advise in this situation. 

✅ The general requirement for adults is 2.4 mcg/ day. The dose you take is much higher as the body can absorb only a small amount. Daily doses may be better. Children 1.2 -2.4 mcg/kg depending on age. Doses tend to be higher in pregnancy and breast feeding. Speak to your health professional.

✅Vitamin B12 (a water soluble vitamin) supplements are cheap and safe and doesn’t need the close attention if one is trying to supplement only through foods (2-3 times a day of fortified foods).

✅Even omnivores (40%) can be b12 deficient, especially with increasing age. Supplementing after the age of 50 in this group is recommended by some experts. 

✅ Medications like metformin used in PCOS or type 2 diabetes can deplete B12 stores over time, so supplementation is important.

Like most health professionals, I have really tried during the course of my career to deliver the best medical care I could to help patients get better. 

However, it is also important to be taught the science of application of lifestyle and nutrition alongside medications and surgery and getting to the root cause of any disease process. There is enough scientific evidence that lifestyle and nutrition play a very important role in preventing and managing many health conditions, including reproductive and hormonal health.

Applying all aspects of lifestyle alongside western medicine as needed can really make a great difference to the overall outcome to the health of the patients we treat.

It really doesn’t have to be one or the other. I wish more medical schools and more doctors would embrace this more whole heartedly. 

This is the approach applied throughout when @rohinibajekal and I wrote 
Living PCOS Free.

I would love to know what you think of our book. It is helpful even if you don’t have the condition of PCOS as there is a lot in the book that can apply to you, whatever your age or gender.

REPRODUCTIVE FREEDOM  is a human right, not a choice. PLEASE PLEASE AMPLIFY

The news about abortion laws being possibly overturned by the US Supreme Court has caused me turmoil.

It is my duty to speak up as an ObGyn who has seen pregnancy-related horrors during my long career and the impact of what happens when choice is taken away.

Pregnancy is a dangerous business, both a mental and physical challenge for many, with disastrous consequences when things go wrong.

Surely, anyone with a uterus should be allowed to decide for themselves whether they wish to carry a pregnancy or not.

If we take away safe choices, they are backed into a corner and left with no choice except an unsafe one.

Let me share with you just one of the many events I have witnessed. 

It has left an indelible scar in my heart and has influenced my practice deeply over the last 35 years. 

In my first year as a young ObGyn resident in India, I was called in the middle of the night to an emergency.

A 18 year old law student carrying twins around 4 months pregnant was in a critical condition. She was cold and clammy with a weak thready pulse and her blood pressure virtually unrecordable. 

I immediately called for senior help who arrived right away to resuscitate her. 

She was bleeding profusely from the vagina. My heart sank as my fingers felt slippery tube like structures coming away in my hand. 

Even though I had never come across anything like this, I knew I was feeling her intestines in my fingers.

Her uterus had been perforated in a back street abortion and the bowel pulled out of the womb in a botched job. 

To avoid shame to the family, her father had dragged her for a backstreet abortion to get rid of the pregnancy. These usually go horribly wrong with terrible outcomes. 

Sadly, the young woman did not survive despite a ten hour operation and died from septicaemic shock.

A life incomplete, her potential never realised, all because someone other than her thought they had control over her body.

What if she had been me or my sister, my daughter, my friend?

Even when the foetus or woman isn't in danger, it should ALWAYS be the WOMAN’s choice.
